2011-2012 Registration Information

After you accept  a spot on our team, you are required to start your registration online using this link.  The online registration will allow you to pay the  required $100 registration fee with a credit card, to secure your spot.  A registration is complete once we receive all signed, notarized paperwork and copies of birth certificates (new players only).

We hold a signing day which is an official ‘welcome’ to our organization.  At this event, you will meet all coaches, administration, other players and parents; turn in your complete registration and make payment for the upcoming year.

You will need to fill-out the following paperwork to consider your registration complete:

1. Signed 2011-2012 Registration Form.
2. Player Waiver
3. Code of Conduct
4. Notarized SCYS Medical Release form
5. Photo Release form.
6. Automatic Draft Authorization form.
7. 3 copies of birth certificate (new player only)
